To the best of your knowledge, why were you blacklisted: I got told it detected Bhop
List any times you have used methods to change or hide your IP: I have never.

The other accounts have been hacked so I don't use them.
Prior to this, have you ever been warned/kicked/banned/blacklisted: I have from what I remember received less then 10 warnings. No Kicks, No Bans and no Blacklists.
What will you do to stop this from happening again: I am not really sure what to put in this as I didn't have scripts or cheats for bhop so I can just continue to follow the rules like I was. I will make sure not to try and bhop naturally because it could trigger it again.
Why should you be unblacklisted: I should be unblack listed because I wasn't using any cheats or scripts, most of SL and SSL who were looking into it, agree with me and I know for a fact I dont use cheats because I have already learn from the past when I was like 14 years old on cs:go.. I was in the staff room getting skipa ready for his mod test and I got kind of bored so I did a natural bhop which I did hit good strafes and got good speed which it could of been that triggering the anti cheat. But yeah my pc has been looked through by screen share and by scan and I Don't have a reputation for doing severe stuff like this.
Even after the blacklist I have been trying to stay connected to the server and help people with problems they have within the server.
