To the best of your knowledge, why were you blacklisted: I've been blacklisted before for some unknown reason but it was an error. I'm not sure what caused it this time because I took a break from the server for about a week to do IRL stuff and focus on my life

Prior to this, have you ever been warned/kicked/banned/blacklisted: Yes I was falsely blacklisted once before and I got it appealed
What will you do to stop this from happening again: Not leave the server for an extended amount of time
Why should you be unblacklisted: I feel like I should be unblacklisted because I was met a lot of cool people on the server and I'd really like to play it again
