What will you do to stop this from happening again: I regret for the incidents that took place and for any inconvenience caused. In the future, I will not remove jars, flasks or any chemicals from the fridge unless I am positive that they are needed for a patient or a certain treatment. In the future I will abide by the set procedures and will consult if I am not sure.

Why should you be unbanned: I know that my behavior may have raised some issues and for this I am sorry. I have thought about my mistake and I have gained experience from it. I shall ensure that I adhere to all the required procedures and make sure that such an occurrence does not happen in the future. I want to be a positive contributor and be a good doctor in this environment. In my future, if I am given a chance, I will show that I can follow the procedures and be responsible for my activities.

After speaking with the staff member who banned you about the situation regarding your ban. I have decided to deny your appeal. While we do appreciate you giving back most of the chemicals, you failed to give all of it back. Additionally, you also lied to the staff member when confronted.

You may not re-appeal this ban due to the length of it.​
