To the best of your knowledge, why were you blacklisted: I'm not sure why I got blacklisted. I joined the server and that's when I was told about my blacklist.
List any times you have used methods to change or hide your IP: I use a VPN from time to time, not sure if that was the reason why I was blacklisted.
List any alt accounts: Don't have any.
Prior to this, have you ever been warned/kicked/banned/blacklisted: I haven't received any punishments at all, even after resigning from staff.
What will you do to stop this from happening again: Not use a VPN when playing on the server.
Why should you be unblacklisted: I have did nothing to warrant a blacklist, I think this was a mistake.
