To the best of your knowledge, why were you blacklisted: Totally unsure, it's a recurring problem for me to be randomly blacklisted. My best assumption might be me having a VPN that kept connecting without my knowledge until about 3 days ago. No other explainable reason however.
List any times you have used methods to change or hide your IP: I mean to my knowledge I hadn't until I discovered it a few days ago

Prior to this, have you ever been warned/kicked/banned/blacklisted: Had none active at the time of being randomly blacklisted
What will you do to stop this from happening again: I've deleted the VPN and I didn't do anything else to deserve a blacklist
Why should you be unblacklisted: Had a good and consistent run with the server before randomly getting banned got staff Overseer Assistant A-1 ISD Commissioner GSD Captain all in a couple months then took a break and came back blacklisted. I just want to get back and enjoy the server again so I plead you just accept this ASAP because it's a genuinely annoying and consistent problem which I've previously been able to sort by DM'ing a CS+ only problem is this time I'm yet to receive a response so I've filled out an appeal.
