To the best of your knowledge, why were you blacklisted: after speaking with snake in a ticket i figured out i was banned for cheating, the only possible explanation i could see for this is that i joined a server yesterday and one of the addons may be conflicting with the anti cheat.

Prior to this, have you ever been warned/kicked/banned/blacklisted: i have been warned a couple times.
What will you do to stop this from happening again: i would fix the conflicting issue.
Why should you be unblacklisted: i do not and have never cheated.
