To the best of your knowledge, why were you blacklisted: Possible mistake? I haven't done anything that would get me blacklisted. Haven't even been on the last 3 days.
List any times you have used methods to change or hide your IP: I've used a VPN one time, but I deleted it and never played GMOD with it. I don't have any VPNs or IP changing stuff on my PC.
List any alt accounts: I have an old discord account 'niko.edu' that I used for my roles and stuff. That has since been abandoned (lost access to it)
Prior to this, have you ever been warned/kicked/banned/blacklisted: I haven't gotten a single warn/kick/ban/bl before this.
What will you do to stop this from happening again: I'm unsure why I was blacklisted, so I don't know what I'd do to stop this.
Why should you be unblacklisted: The blacklist is false, as I haven't done anything wrong that would warrant a BL (Edit): I have had a clean punishment record for over 2 years, I wouldn't randomly start doing stuff that would get me blacklisted
