To the best of your knowledge, why were you blacklisted: I have a glitch were my IP would some reason change to the IP of someone who was blacklisted. This happened like 6 times over the span of 3 weeks before I left the server. This keeps happening and I do not know the reasoning behind it.
List any times you have used methods to change or hide your IP: To my knoledge I have never changed it before
List any alt accounts: None
Prior to this, have you ever been warned/kicked/banned/blacklisted: I have been warned, 3 times. This was when I first played the server, and I have never been banned to my knoledge for any reason.
What will you do to stop this from happening again: I don't know, I can't control it for happening. This is just a thing that happens randomly every couple of days or so.
Why should you be unblacklisted: I'm trying to come back to the server, aswell as this is a constant false black list that has repeatly happened in the past.
