PAC Requested:

During a psychological evaluation with 076, one of the consultants accompanying asked a question about 076's sibling which threw 076 into a rage state, unluckily cuddles was looking at her notepad a bit too close to the cell, allowing 076 to have the reach to cleave off the arm. The medical staff accompanying her staunched the bleeding and due to the accident being work related was funded for a new limb to continue work for the foundation as a whole
