Yes for severe toxicity and LTAP. I have since learned from my previous actions and will ensure that this behavior does not come up again.

What makes you suitable for SCP-096?:
I have been on the server quite a bit for the last few months and have a very good understanding of how the shy guy works. Had done a cross test with 096 while roleplaying as 049 and I am very aware of his behavioral patterns as well as his mannerisms.

What is the usual behaviour of SCP-096 when pacified?:
When pacified The shy guy will face a specific wall in his containment chamber and will make any attempt to cover his face so that no Foundation member or Civilian will catch a glimpse of his face inside his chamber.

When breached, where is SCP-096 allowed to go? Can SCP-096 beeline straight to D-Block?:
The only way that 096 can target anyone in general is if the individual had caught a glimpse at its face. The only way 096 can enter d block is if someone inside of Dblock had seen a picture or directly caught a glimpse of its face.

How is SCP-096 re-contained? Please describe all steps of the process:
First a bucket would have to be placed on the head of SCP- 096 and then Foundation members with the proper clearance level would then need to use a Containment beam on the specimen to further pacify him. SCP restraints would then need to be bound on the SCP and then would be dragged back to his cell.
