Have you received any kicks/bans/warning? and why?:
I have received a few warnings. I believe two for RDM and one for trespassing as a Gun Dealer. The Gun Dealer one was when I was very new and regret it. And one of the RDM warnings was for killing someone ear raping me during RP with the Syrian National Anthem.. who later renamed themselves the N word and got banned.
Do you have any previous experience as a staff member in a community?:

On GMOD this would be my first experience with Moderation but I am very keen to learn.
Off of GMOD, I have moderated game servers on other games such as Mount & Blade Warband, moderating and being a part of different teams and my highest rank on there being in a Senior Role being responsible over Team Discipline, Warnings & teaching new Moderators.
In my spare time I also moderate a large discord server as an Administrator (Team Lead) of a server with 600,000 members.
Update; This server is now at 650,000 Members, and we are currently in the process of hiring new Helpers/Onboarding.
Do you have any previous experience with serious roleplay?:

Yes I do. On the game I mentioned previously, Mount & Blade Warband, the theme is more medieval. As the game has built in swordplay and faction like aspects. The serious roleplay I was involved in there involved being creative to be rewarded, such as being a Smith for a Lord, or a Steward for a Merchant. I enjoy building complex characters when the facilities are in place for it. Or developing scenarios for myself and others to be involved in.

I have broadened my horizons here on SCP RP, as an Omega-1 Captain my responsibilities have not only ranged from managing my own regiment as best I can, by assisting my Commanders; but I also assist a lot in the IA and Intelligence Department. Handling different situations that occur and usually defusing the bad ones. I like to think I'm a good speaker and quite charismatic when I need to be. I fit well into most scenarios and can pick up almost any role I'm given.
How many hours can you be on everyday?:
1 -2 , I work part-time and have other commitments, but I will give as much time I can to Moderation. I hope to learn from my position here, and use it to increase my own experience with Game Moderation to help me become better at it.
Why do you want to become a moderator for our server?:
I love the theme of the server and the Players. I play it a lot and feel like I can be of good assistance and be utilised well as a part of the team. I work well within Teams, get on with people easily and can adapt well.
I feel becoming a Moderator here and progressing would help me better my own skills in Moderations and working with people teams in a different medium of server that I'm used to.
What do you think you would be able to help us with on the server? Why are you special over the other applicants?:
I have experience working within teams previously. I've moderated on Game Servers before, Discord Servers before, Forums before.. I can adapt well and will fight to get better at any task I'm given. I want to help out as much as I can and increase my efficiency at it in the process. I hope I can offer the team a good team player that can grow myself as much as I can help the team grow too.
