Have you received any kicks/bans/warning? and why?: I did have a warning as a Surface SCP cause I kept jumping avoiding getting cuffed but stopped as soon I heard it was against the rules. this was like 5ish months ago.

What is the usual behaviour of SCP-096 when pacified?: Will mostly be sitting in a corner blocking his face and crying to have no one look at him since he hates when people look at him.

When breached, where is SCP-096 allowed to go? Can SCP-096 beeline straight to D-Block?: To my knowledge anywhere he wants only to kill the target that looked at him to try and kill them, and no he can not beeline to D-block unless someone saw his picture and he is bucketed?

How is SCP-096 re-contained? Please describe all steps of the process: Someone needs to put a bucket on his head to keep him from getting enraged any further, needs to follow him to hopefully kill his target so he is no longer enraged, restrained and dragged all the way back to his containment room with the doors closed. To be honest I never really got to help with recontaining him so I am assuming this is what happens from other SCPs.
