- 5/14/23 - Micspam [A] - First time on the server and didn't know that it was wrong, started playing soundboards of music.
- 6/27/23 - RDM - Killed the wrong D-class from catwalk because I thought they had a gun.
- 5/11/24 - ERP [A] - This is my only warning I don't remember getting. No reason given. (Going to have staff review this warning)
- 12/5/24 - FearRP - First time back after a 3 month break and forgot about the rule. I ran when under gunpoint by CI.
- 1/3/25 - Hitbox Abuse - On SCP-096, I walked out and sat on the flasks because I didn't see them, causing the D-class to see my face and trigger me.
- 7/5/25 - Metagame - Used 3rd person when I hide from a hostile civ, then called a sit for it. The clip I sent to the mod showed me 3rd person peak.
Why are you applying for Medical Consultant?
- I would love to get Medical Consultant for a few reasons. I would like to gain access to more roleplay opportunities to give me a better understanding on how to improve my roleplaying capabilities. I believe I will be great for this job as I am very active as Sr. Doctor and Combat Medic, which would allow me to help with any matters that may be during High or Low hours. I can and would also help to guide new members and members who are confused about what to do on the policies, trainings, or tryouts. I also enjoy being around and helping the medical leadership team whenever I can. This job will help me learn more about the department and how to better my medical operations.
What makes you suitable for Medical Consultant?:
- I believe I'm a greatly suitable for Medical Consultant because I have had a couple CL4 jobs, which have all helped me learn some skillsets that will help me with being a Consultant. I have about a year's worth of experience as a Gensec Captain from March 2025 to October 2025, then also from December 2025 to March 2026. As my time as Captain, I've gained better leadership and management skills within my old department. It has also given me better knowledge of using Google Docs & Google Sheets, and also how to manage Sub Regiments within a department, with being STT/TRT, where I was the LT and then COM for a few months, and when I was an assistant & assistant manager of G&I when it was active and a thing, where I learned to help people with problems in Gensec. I also have some good experience as an Ethics Assistant from June 2025 to November 2025, where I learned how to manage In game documents and take complaints better than I did for Captain. I have also learned a really good understanding of the Code of Ethics, Code of Conduct, and Foundation Legal Codex from being an Ethics Assistant. Being a Combat Medic and Sr. Doctor, these roles have given me more experience outside of just Gensec and Ethics. This has been my current rank for the past month. Thanks to the people within the department, they have taught me how to do medical procedures, handle hazardous objects, and to work while having fun.

What are the responsibilities of the Medical Consultant in RP?:
- Giving tryouts to people who don't have their Medical License or Combat Medic Whitelist.
- Helping with Medical when requested for Tests.
- Perform Surgeries when needed.
- Perform evaluations on site staff who may require them.
- To help supply Medical staff with the right chemicals that are used in day-to-day work.
- Help people who have any questions and to train new members of the department
- Uphold an ethical manner within the medical department.
- Help the Directors and Consultants manage the medical members.

Please explain an in-game medical procedure of your choice:
Physical Evaluation
#1. Grab the "Physical Evaluation" Document from the Medical Website. (Home>Handbook>Templates>Personnel Physical Evaluation.dat)
#2. Write your name and the date down on Page 1, and change the title of the Document to "Physical: [Insert Name Here]".
#3. On page 2, ask the patient for their name, age, rank, and the reason for the Physical Eval.
#4. On pages 3-4, ask about the following questions on the Document to determine how the patient normally treats themselves and if they feel any current fatigue/illnesses.
#5. To start page 5, start to perform the Physical Eval by checking the patient's blood pressure by doing a 130 roll (/roll 130) and a 90 roll (/roll 90), then writing down the Category, Systolic, and Diastolic. (Example: Elevated 123/87) (123 being the Systolic check and 87 being the Diastolic check)
#5.1. To get the Category, you must first check the patient's Systolic. To check Systolic, roll 130 (/roll 130), and to check Diastolic, roll 90 (/roll 90).
#6. To check the patient's mouth and ears, you just shine your flashlight into the patient's mouth and ears and roll 100 (/roll 100).
#7. To check the patient's heart and lung auscultation, roll 2 100s (/roll 100), then afterwrite Good (50+) and Weak (49-). (Example: Good, 68)
#8. Ask the patient if they've ever received ECT.
#9. Shine your flashlight into the patient's eyes, and have them follow it with their eyes. Then roll 100 (/roll 100) to check how the pupils respond.
#10. Have the patient stand next to the measuring stick in the treatment room.
#11. To check the patient's leg reflexes, just roll 100 (/roll 100) for both of the legs.
#12. Ask the patient to lay down in the Infection Scanner, then write down any infections that may be in the patient's Mouth, Ears, Eyes, Heart, Lungs, or Skin. You should also ask them if they have any known infections or diseases.
#13. Ask the patient if they have any pain, then have them rate it on a scale of 1 to 10. (No pain would be 1)
#14. Have the patient lay down in the X-Ray machine, then write down any fractures.
#15. For the eye exam, have them stand behind the Yellow and Black line on the floor of treatment room. Ask them to please read the line that is most clear to them, and write down what they got wrong (If any) and the numbers next to it.
#16. To end the Physical, ask them if they have any more concerns or additional notes they would like to talk about before ending. After, write a short summary on how the exam went, and if and why they will need medication or a work exemption.
#17. After, print out the Document and sign it. Then go into the Psych-Eval room (Right across the Training/Meeting room) and log it in Script.net at (/FOUNDATION/SITE_65/DEPARTMENTS/MEDICAL/EVALUATIONS/[Insert Department]/PHYSICAL_WELLBEING/) and put the physical document in the filing cabinet,

Tiffany was born on July 30, 1999, in █████ Pennsylvania. She lived a somewhat normal childhood growing up in a poor family. At the age of 14, she was forced to get a job to help her parents keep their house. She abandoned her family when she was 17. When she turned 18, she joined the United States Army. She served a 4-year contract that reached the rank of Sergeant. During her time, she mostly worked with documents, approving new recruits and discharging them. After her 4 years, she joined her local police force as an investigator when she was 22. Most of her time, she sat at her desk playing War Thunder since she was in a small town with little happening.
On 4/23/22, there was a call in the Chiefs office. As his face went from enthusiastic to serious as he walked out of the office and called her over. He said that a government agency needed her help with an international case. She agreed because the town was a bit lonely. She left the town the next day for a far town where she would be picked up. When she got there, she was a bit confused when she saw no plane or car, and two heavily armed personnel took her bags. They asked her a her questions to identify it was her. She could barely hear them due to the voice filter they had on. Once they confirmed it was, they walked her over to an empty runway. At the end, there were about 4 more fully masked soldiers. She was still confused because it was late at night, and the runway was empty and completely silent, which was weird since there was a highway only a few thousand feet away. When she got to them, the people behind her grabbed her arms and stabbed a needle in her neck, which quickly paralyzed her, making her fall asleep.
When she woke up, she was handcuffed to a chair in a dark room. She felt that she was no where near where she was before she got knocked out. About 20 minutes went by before a person in a black and gray uniform walked in with three patches on them. A weird logo with a person on it, one that says "L.E.", and one she could barely see because their head was turned, it said "M.T.-". They didn't use a voice filter but asked nicely not to scream or fight, that she was not in danger, and could be released at any time. She asked what was happening when they said just an interview. Right then, the door opens, and a person in a tan brown suit walks in. They put down a briefcase with a recorder. They said, "What's your name and birthdate?". I answered. Right as I finished, the lights cut off, then turned on a few seconds later with a loud noise and voice coming through the speaker. It screamed, "Code 5 Containment Breach." They didn't move, though, as one of them pulled their pistol and shot the speaker off the wall. They talked then, after she was told. "You have two options. You can go back to your normal job, living a boring life, or just us. You probably won't see the town again, but you will do a lot more for the world than sitting at a desk playing games. I'll give 10 minutes to choose." He was about to walk when she yelled for the job. Then her training began.
