The janitor one is awful. I'd only like this if it only applied if you could see the thing that was broken (i.e. shorter range, and not shown through walls) - I'd support that, but not having it be exactly like the janitor one which makes no sense.
I don't think it would actually be nearly the issue people say it would be, but I do think that any concerns could be covered by just restricting crediting to:

Makes sense for their job - the entire point of the job is to find combative issues around the site, so they should be able to call it like this when they succeed.
The... same systems. To be gagged you have to be in restraints, so you couldn't use a gun anyway. To me, this just seems like a cheaper option but which is useful in less situations (can't strip cyanide, can strip weapons; you get restrained before you get gagged; etc.). It also just makes sense...
You can already commit suicide using cyanide, which is probably an even more brutal way to die, given you essentially fill your insides with acid.
